New Patient/Referral Requirements



 We require the following information when referred/referring to our office. Obtaining this information in advance allows for efficient and timely scheduling/billing. If this information has not been forwarded to us, we will kindly ask for this to be done prior to scheduling.

 

  • Patient’s most recent history and physical, reason for referral (including supporting notes) recent labs and/or imaging.

 

  • Patient demographics including address, phone number(s), and current insurance information.

 

  • If a COLONOSCOPY is being ordered; it is vital to have a record of the most recent colonoscopy performed along with any pathology (if taken). This ensures us how to schedule and bill for your procedure.  

 

Once we receive the specified information as above, this will be reviewed by our staff and we will reach out for scheduling. Please allow at least 48 hours after referral has been sent prior to calling the office unless your appointment needs are urgent.  

 

NOTE: Please do not send multiple referral requests if your patient states they have not been contacted by our office. Please call the office to confirm that we did in fact receive the information.
